Medical Certificate Instructions

1. Download and print the official SKM 2026 Fit-to-Run Medical Certificate form below.
2. Bring the printed form to a licensed physician. A doctor from either a public or private clinic or hospital may examine you and sign the form.
3. The examination and certificate date must be August 19, 2026 or later. Certificates dated earlier than August 19, 2026 will not be accepted.
4. Make sure the physician signs the form and supplies their name, PRC license number, and clinic or hospital details.
5. Upload a clear scan or photo of the completed and signed form. Accepted formats are JPEG, JPG, HEIC, PNG, and PDF, up to 5MB.
6. Keep the original signed form. The organizer may request it for verification or race kit claiming.

No approved medical certificate, no race kit release, and no participation in the 21K or 42K category.
